We read with great interest the article on "predicting disease severity of necrotizing enterocolitis (NEC): How to identify infants for future novel therapies" by Troy et al. published in the first issue of 2014. The article is very interesting and informative. However, we would like to add some points on radiographic predictors of NEC.

Radiologist plays a critical role in evaluating patients with NEC. Once the diagnosis of NEC has been established, interval plain abdominal radiographs (6-24 hourly) are essential for appropriate follow-up. Resolution of NEC is characterized by gradually normal appearance of dilated bowel. Ominous signs in abdominal radiographs are persistence of dilatation, change other than in the normal direction, change from generalized to localized dilatation and fixed appearance of loops. The disappearance of intramural gas and portal venous gas is not always associated with clinical improvement, and these are thus poor indicators of progress. [1] We would like to contribute by showing a classical radiological image of NEC [Figure 1].
